When making a decision, it is easy to remember only the final result. A short decision record preserves the conditions of the moment and keeps the outcome from replacing the process in hindsight.

Write down the conditions first

Record what you knew, the options available, and what remained uncertain. It does not need to read like a report; it only needs to help your future self understand why the judgment made sense then.

Review with kindness

The point of a review is not to prove that your past self was right or wrong. It is to recognize which signals are worth trusting again and which assumptions need to change.
